â€Å"Cathedral† The story â€Å"Cathedral† demonstrates that lack of sight does not necessarily prevent one from perceiving things as they are, or live their life to the fullest. In the story, a middle-age blind man, who is a friend to the narrator’s wife, and used to be her boss at one point, visits the narrator and his wife. The narrator has never interacted with blind people before, and all he knew about blind people was what he had seen on television. Blind people are stereotypically portrayed on television as slow moving, dull people, who never laugh. Based on this perception, the narrator was reluctant to meet the blind man and doubted whether they were going to connect. This is evident when the narrator states, â€Å"I wasn’t enthusiastic about his visit. He was no one I knew. And his being blind bothered me† (Carver 1). Heart disease, stroke, kidney disease, blindness, nerve damage, leg and foot amputations, and even death are among the results that a person can wind up with if they do not manage this disease appropriately. Diabetes occurs in two stages with the initial one being a warning that you are at a high risk of being diagnosed with type 2 diabetes. The first stage is known as prediabetes, this stage occurs when your blood (glucose) sugar level is higher than normal but not yet high enough to be type 2 diabetes. At this point, if drastic lifestyle changes are not made by a person, then it will inevitably progress to type 2 diabetes. It was the sickly stench of dried blood. In the room five men were seated around a large table, glaring at each other. Brandon Cole walked through the darkness. After finishing work he had decided to take the short route home. Brandon wrapped his arms around him. It was cold. The sound of Brandon’s footsteps echoed through the narrow road. Brandon could see nothing – it was pitch black, he squinted to try and see through the darkness. Brandon continued walking down the stone road, he felt something squelch beneath his feet, he muttered irritably to himself and grabbed his lighter from his pocket. He flicked the lid back and the lighter burst in to light, like a tiny explosion. The small flame did hardly anything to improve Brandon’s vision against the darkness. Brandon bent down placing the lighter by his foot, squinting again. Brandon began walking again, he turned a corner and heard a small noise behind him, and Brandon spun around quickly. Probably that flaming dog, he thought. In the circle two men were fighting, the sound of blood splattering on the floor could be heard above the cheering. The sound of clenched fists smashing against bone could be easily heard. Boran smiled. Boran walked into the middle of the circle. The cheering stopped suddenly. Boran smiled, kicked off his shoes and took his tie off. He pointed to a stocky man from the crowd. Boran studied him carefully; the half-light made it difficult to see. From what he could see Boran saw the man had blonde hair; he had grey eyes and looked pretty strong. It was Thursday, Boran lowered his head – he was leaving Friday, he wondered if something was going to happen before then. He hoped it wouldn’t, Boran looked at his watch -Friday couldn’t come soon enough. Beth and Brandon were sat in the car; the drive would take about ten minutes. Luckily it had become quite cloudy and it was beginning to get dark, the light hurt both Brandon and Beth’s eyes. Beth rubbed her stomach, she was aware of a pain the came every now and then; it was like someone kicking her there. She frowned and brought her hand away slowly. Brandon cast her a cursory glance but focused on the road when he realised she was ok. Beth looked at the clock, it told her that it was 6:30pm. Tyler checked each of his team again, they were ready, so was Tyler. ‘Mr. Freeman, what time do we leave, sir?’, the voice came from the youngest looking of the men, Tyler reasoned he could be no more than twenty-two ‘Ten minutes’ Tyler said. The van was small and the compartment where Tyler and his party were sat had everything, computers, video cameras, and phones. Tyler looked at the machinery and he spotted something. He bent down to see the object and he finally realised that it as a walkie-talkie. Tyler grabbed it and noticed many more lay beneath a small cloth, he picked up five and passed them to each man in the group ‘These might come in handy’ said Tyler in an unmoving tone, he attached the two-way to his belt and checked his watch, it was 6:40pm. Tyler opened the back door of the van ‘Let’s go’ he ordered. The five other men followed Tyler. Across the street a car pulled up. Boran was stood still, behind him were the rest of the nest. Stood next to him was the American man, Boran looked at him ‘Matthew, you must protect me. Ok?’ The American man looked at him and nodded, he looked forward again and shut his eyes a bead of perspiration ran down his face. One word roared through his mind TRAITOR! He noticed that some of the men had begun to move, they had been standing for quite a while, Boran clicked his fingers to silence the men again, it worked. Another knock came at the door and Boran felt his left knee begin to shake. The door crashed down and made a huge noise, it was intensified by the echo that it caused. Both Boran and Tyler’s mouths dropped open as they saw each other ‘Master’s of the opposite sides’ Boran said trying to smile and look unafraid. Tyler glared at him ‘Armageddon’ he whispered. In an instant Tyler raised his gun and he fired off three rounds, the recoil hurting the palm of his hand. The first bullet hit Matthew in the chest and he screamed in agony as the bullet tore through him, he managed to gurgle two words ‘I’m sorry’, blood began to spill over his lip. The second bullet hit him in the side of his face and Tyler heard the crack of bone then saw his face rip off because of the force of the bullet. The third shot missed and hit the wall blasting a huge hole in it; plaster and brick flew off in chunks from the wall hitting Boran and some of his followers. ‘Boran, I could kill you so easily. Just one shot.’ Tyler chuckled to himself/ Boran felt the anger rising within him and he clenched his fists until his knuckles turned white with rage ‘Tyler, you know what is supposed to happen when the two masters meet’, Tyler glared at Boran and finally stated ‘A fight to the death’. Boran smiled ‘That’s right Tyler. What Else?’ Tyler stared at Boran for what seemed be an eternity and managed to say ‘Armageddon.’ Boran smiled and whispered ‘Who too Tyler?’ Tyler stared down at the floor ‘To the side that loses’ Tyler met Boran’s icy glare ‘But it’s only hearsay, it’s never been proven!’ he said this defensively, as if it would make Boran reconsider. Fight me to the death.’ Tyler raised his gun and shot at Boran, the shot hit him in the elbow and he bellowed in pain as his lower arm was ripped away, he fell to the floor gripped his arm, it was pumping blood. Boran screamed one word ‘Attack!’ The entire army ran towards the five men. Tyler was about to fire again when he felt a large hand grip his shoulder, he turned his neck and raised his arm. It smashed the vampire in the face and it fell backwards. Tyler turned around and raised his sword, he brought it down with an amazing force on to the vampire’s head. Tyler heard the satisfying crack of bone and felt the blood of the vampire hit his face. He ripped the sword out of the vampire’s skull and swung it around him, it hit another vampire and chopped it completely off at the waist, the legs remained upright for a second then fell into a heap on the floor. Tyler jumped into the shadows and gripped the two-way. He pressed the small button on the side of it. Tyler spoke calmly into it ‘Requesting backup in Caldicot Road warehouse. Quickly, this is a code blue emergency’, there was a hiss of static then a voice from the other end ‘Ok Tyler, I got that. Over and out.’ Tyler clipped the two-way back on to his belt and reached for his gun. He could hear the gunfire of his partners. ‘At least they aren’t dead yet’ muttered Tyler. The gun was light and Tyler looked at it, he knew the technical specs of the gun very well and eventually found the button he sought. He pushed it and smiled, the gun was programmed to rapid-fire. Tyler stood up and raised the gun. He pulled and held the trigger. Brandon and Beth heard the gunfire from the warehouse and looked at each other. Brandon placed the key back in the ignition and started it up. Beth locked her door; she looked out of the window and saw people running from the gunfire yet nobody ran out of the warehouse. The deafening sound of gunfire drowned out the sound of screams and Beth closed her eyes, trying to ignore the sounds. The car began moving and Beth felt a sharp pain in her stomach, was it a warning to go back? She ignored it and the car drove on. Tyler felt the recoil of the gun against the palm of his hand. He gritted his teeth and shut his eyes. He moved the gun from side to side hoping it was hitting something. Tyler heard the click of the gun showing that the magazine was empty. Tyler quickly ducked and slammed in another one. Immediately he stood up and began firing again. Bodies were scattered across the floor and blood was spattered on the walls and ceiling, it was pumping from the bullet-ridden bodies oozing on to the floor. Tyler felt the gun stop firing and ducked back into the shadows, he reloaded but stayed down for a second this time. He heard sirens in the distance and smiled. Boran rubbed the perspiration from his pale face. Was it over, or was it just beginning? Boran clenched his only fist and swiped at the mirror that was in the room, as it cracked he saw the room reflected at least twelve times, but no reflection. Boran walked out of the room, sweat was soaking his back and armpits. One word raced through his mind and refused to leave. Armageddon. The firing had stopped; the agonising wails of pain had stopped. Tyler came up from the shadows and saw twelve men; it was the back up team. ‘Hey guys. They all dead?’ Tyler pointed in the direction of one of the vampires. The twelve men of the backup team looked at Tyler ‘Hey Tyler, what the hell happened here?’ Tyler looked at the men ‘What does it look like?’ Silence. Tyler looked at the spot where Boran had been lying ‘Oh Christ’ he murmured. Tyler heard the massive crash of the door and raised his gun. He fired three rounds at the figure that stood in the doorway and each shot hit it. The figure began walking towards Tyler. Finally the dim light spread across the beings face ‘Boran’ Tyler muttered. Boran smiled ‘Hello Tyler’ he whispered. Tyler threw his gun away ‘Fine, a fight to the death!’ Both men ran at each other and as Boran lashed out with one powerful fist Tyler fell to the floor, ducking from the fatal blow. Boran cursed and stopped himself from running. Tyler pulled himself up and grabbed one of the wooden steaks from his belt. Tyler heard Boran running up behind him and shut his eyes tight. In one swift movement Tyler was up and had spun his whole body round, the wooden steak came around in his hand, it ripped through Boran’s skin, muscle and bone and finally pierced his heart. Boran felt one moment of searing pain and then it subsided to nothingness, Boran could feel himself slipping away in to unconsciousness. In to death. Boran opened his mouth and blood began to trickle from it, running down his chin and splashing on the floor, like tiny crimson pools. Boran tried to speak but found it almost impossible, he could only focus on one word. He coughed up more blood and tried to speak again ‘A†¦Ar.’ Tyler glared at him coldly and finally whispered ‘Armageddon.’ Darkness enveloped the earth; it was like the entire world had been covered by a huge blanket. People stared up in both disbelief and fear. Vampires everywhere began to fall, wither away and slide in to oblivion, into an eternal sleep. The darkness seemed to grow greater and an icy wind ran chills through the soul of everybody. The darkness seemed impenetrable. The body of Boran fell to the ground and Tyler opened his own eyes. An icy chill ran through him. It was over. ‘We’ve won. The world is ours.’ The twelve men looked at each other confusedly, not understanding what Tyler was saying. Tyler looked outside and the darkness began to dissipate, blue skies took over the world again. The darkness faded away into nothingness. One thought was on Tyler’s mind. Who had sent him the note? The thought hit Tyler suddenly and he gasped. He bent down at the body of Boran and pushed his hand in to Boran’s bloodied pocket. Nothing. Tyler walked over to Matthew and searched his pockets; inside one of the trouser pockets was note, it was written in blood; Tyler smiled at the words scrawled on it. I AM A TRAITOR. Beth was different though. She had been pregnant. When the doctors had found her a case had taken place, Beth was hooked up to a life support machine to keep her baby alive. Although her brain was dead technology could keep her body working normally. She had been lying on the bed for two months and the doctors had taken tests that showed the baby would be perfect. The tests had proved to be right, the baby had been born less than thirty minutes ago and it was fine. Beth Cole had given birth to a perfectly healthy baby boy. The doctors were stood around Beth’s bed waiting for the leading doctor to pull the plug that would stop her body from working, finally he did. The body of Beth Cole was taken the morgue. There are four types of privacy including physical privacy, informational privacy, decisional privacy and proprietary privacy (Siriwardhana, Adikari, Jayaweera Sumathipala, 2013). Confidentiality is a right to restrict the disclosure of the personal information. While providing mental healthcare to a Tangata Whai Ora mental health patient, I faced ethical dilemma of privacy and confidentiality. The patient has reported me certain information that is potentially harmful to himself or someone else and thus I faced dilemma as a service worker in reporting the information. Moreover, a psychiatric patient does not have a stable frame of mind for the information to be truthful. I faced ethical dilemma in reporting that sensitive information to doctor because the patient was not willing to share the same with the doctor. He has shared with me only out of sheer trust and believed th at I won't share the same information to anyone else. Decision making process In the domain of the disclosure of the information, patient's autonomy is at times overridden by the ethical principal of justice and/or nonmaleficence, beneficence (DeKeyser Ganz Berkovitz, 2012). In the decision making process, what I took as a reference is, information can be disclosed in the absence of the patient's information only when the person are falling under a threat to themselves (non-maleficence) and when the information is in a need for providing quality care (beneficence). In my case, the information shared by the patients about his health status was misleading and has no connection with each other. This showed that the patient's was in a complex state of mind and was unable to recollect the thoughts and hence I reported the same to the doctor via narrating his actual quotes. This I need only for the continuity of care of the patient (beneficence) (DeKeyser Ganz Berkovitz, 2012). Maintenance of boundaries in accordance with the wellness plan According to the wellness plan, discussion of the issues in relation of the mental health service seekers must be done with the knowledgeable staffs for delivery of care in a safe manner (Arahura Charitable Trust, 2017). In this case, also the issues that are being experienced by the Tangata Whai Ora disclosed to the doctors as they are most knowledgeable person in the care model who will be successful in procuring quality care. Maintenance of boundaries in accordance with the organizational policies According to the organisational policies, all the staffs are legally restricted to respect the confidentiality and privacy of the Tangata Whai Ora (Kidd, Butler Harris, 2013). This signifies that during the tenure of the work and even beyond the work hours they are required to abide by the privacy policies. In order to remain within the boundary of the organisational policies, I treated the patient with respect and did not discussed his details or personal issues in front of the unauthorised person. What I did is I discussed his concerns with the doctors and only for the betterment of his health. Doctors are the best person to provide quality care to the patients via accessing their present condition. Since it is not possible for the doctors to remain in touch with the patients 24X7, it is the duty of the associated service providers to convey the information to the doctors. Discussion of ethical issues The ethical issue at supervision can be described under the light of the beneficence and nonmaleficence. For the benefit of the patient, in order relief him from the mental complexicities, it is the duty of the nurse to listen careful to very information or stories that is being shared by the mental health patients. Moreover, the nurses are required to generate a strong relationship based on trust with the patients so that they feel comfortable in sharing the information. When it comes sharing the information to the patients to other healthcare professionals who holds higher position in term of hierarchy, the ethical dilemma of confidentiality or privacy can be breached on the grounds of nonmaleficence. The act of nonmaleficence means no harm to the patients and here the nurse is only sharing data with the doctors in order to prevent further mental harm to the patient (Kangasniemi, Vaismoradi, Jasper Turunen, 2013). Ethical issues in accordance with organization policies Organizational policies maintain a strict rule of privacy and confidentiality concern when dealing with mental health patients. However, organization also assures quality care to the patients. In the grounds of providing quality care, a nurse can share confidential data to the doctors (included in the same health care team) in order to improve the therapy plan (Kangasniemi, Vaismoradi, Jasper Turunen, 2013). Addiction Service Users An ethical dilemma: Consent The main ethical dilemma that is faced while providing support to the patients who are suffering from certain kind of addiction (drug or alcohol) is, respecting patients autonomy (Owonikoko, 2013). Principle of autonomy refers to the right of the individual to determine the kind of activities they want or will to participate. The principal of autonomy falls under the category of consent. Decision making process The main decision making process that is being used for resolving the ethical dilemma of autonomy while treating addicted patients is the theory of utilitarianism. According to this theory, utility must always be encouraged as the sum of all the pleasures from an action, minus any suffering of anyone involved in the action. So the principle of utilitarianism always highlights the sum of well-being and ignores any negativity that may arise while working in favour of the benefit (Grant, Kajii, Polak Safra, 2012). I think, even if the addicted (drug or alcohol) Tangata Whai Ora is refusing to give the consent to participate in the addiction recovery service; the therapy can be applied beyond his or her consent. This is because, participation in the addiction recovery therapy, will actually help him or her to enter into a state of well-being. This quality of life will outnumber his or her negative approach in providing the consent in the long run. Maintenance of boundaries in accordance with the wellness plan In order to maintain the boundary of the support worker role in accordance with the organisational policies, I interacted with the Tangata whai Ora personally in order to know the exact reasons behind why he is refusing to participate in the addiction control wellness program (Holmes, 2012). What I elucidated that his concept of well-being is projected towards four basic pillars of life, as discussed in picture below. Figure: Four Dimensions of Wellbeing (Source: Hammell Iwama, 2012) After knowing the same, I tried to incline the effectiveness of the wellness programme to each of the four dimensions in life. I also tried to explain him how addiction control service will help him in stay under the good state of mental health, physical health and spiritual health and all these health status will in turn promote the family health (Hoge et al., 2-13). Thus via remaining within the boundaries of a support worker, I tried to earn the consent of the patient for his participation in the wellness program, without bridging the ethical theory of autonomy (consent). Maintenance of boundaries in accordance with the organizational policies As per the organizational policies, no patient can be forced or allowed to participate in a wellness program beyond his or her consent (Grady, 2015). So in order to remain within the boundaries of the organisational policies, I performed a counselling session with the patient. The counselling session helped in the identification of the dilemma which the patient is facing while participating in the wellness program. So via understanding the root of the problem, I acted accordingly, made him understand the benefit of the wellness program on an utilitarian approach and bridged the gap of autonomy (consent). Discussion of ethical issues The main ethical issues which are coming in supervision apart from the principal of autonomy is, the theory of libertarianism. According to this theory, there must a freedom for everything, like freedom of decision making, freedom of speech, freedom of gender equality and so on. However, the theory of egalitarianism states that there must be equality for all (Moreno-Ternero Roemer, 2012). So under the light of the egalitarianism, the patient here also deserves quality care but is unable to receive the same on the grounds of autonomy and libertarianism. However, his concept of denial is not justified as he is unaware of the consequences of the addiction in his later stages of life so the counselling done during the supervision is justified and will help the patient for fast recovery. Ethical issues in accordance with organization policies Organisational policies give importance of taking consent of patient at every stage of the wellness and recovery therapy (Thiel, Bagdasarov, Harkrider, Johnson Mumford, 2012). So in order to work in accordance with the organisational norms, the patient was counselled in order to obtain the consent and progress the treatment in an ethically correct manner 2.Reflection Reflection review The main service goal for the support workers who are working for an organisation for the betterment of the mental wellbeing of the patient and addiction support regime will patient only. I feel the service workers must work in unison via applying proper ethical theories. They must always consider the wellness of a patient above anything, where contradiction of the ethical principles must also be considered. Moreover, it must also be taken into account that the ethical principals must never be bridged in the grounds of harming the patients. If the ethical principles are bridged only because of providing quality care of the patients then the act of bridging can be justified. Reflection on ethical practices: Self evaluation Practise approach should always directed towards the betterment of the patients and their health. I always try to emphasize that the patient must always receive quality care in order to improve their quality of life. My underlying attitude and philosophy is defined by the ethical theory of egalitarianism. According to this theory every person is entitled to receive quality care and should never be judged on other grounds which may lead to biasness. The grounds include financial, cultural, social, education and habits perceived by the patients. I examine a person solely as a patient who needs quality care for the attainment of proper health and well-being. My personal values and believes state that the principal of beneficence and non-maleficence are interconnected. Beneficence deals with balancing the expected benefits of the treatment against the backdrop of the costs involved and the risk parameter. On the other hand, non-maleficience means avoidance of harm. Several treatments involve certain degree of harm; here the principle of non-maleficence would signify that the possible harm should never be disproportionate to the expected benefit of the treatment. However, abiding the principle of beneficence and non-maleficence may at times can cause harm to a person's autonomy (in some circumstances) (Thiel, Bagdasarov, Harkrider, Johnson Mumford, 2012). For example, it may be required to deliver treatment that is not expected to prevent the development of a future, more serious health related problem. The overall course of the treatment can be extremely unpleasant, uncomfortable or at times can be extremely painful. However, this might cast comparatively less harm to the patient than that would occur previously. So my self-evaluation tells me that my ethical mindset is more inclined towards the ethical principal of beneficence and non-maleficence. Patient's participation in the process of treatment decision, is the principal pillar of health care. This can be related to the highest levels of patient's satisfaction, adherence to the treatment and improved health outcomes. The level of patient's participation is the process of treatment decisions in dependent on the intrapersonal (patient's characteristic) and interpersonal (communication styles between the patient and the care provider) (Morse, Salyers, Rollins, Monroe-DeVita Pfahler, 2012). As per my evaluation, my communication style is strong enough in modulating the patients mindset, while making the patient to actively participate in the healthcare decision making.
